Referral Process
We encourage each prospective resident, and/or substitute decision maker to come and visit Elm Grove Living Centre at any time throughout the referral process.
Contact your Local Health Integrated Network (LHIN) to determine if you are eligible to receive our services. The LHIN will help you coordinate a plan that’s responsive to you and your caregiver or family support system. The LHIN are responsible for authorizing admission to Long Term Care Homes.
