We encourage each prospective resident, and/or substitute decision maker to come and visit Elm Grove Living Centre at any time throughout the referral process.
Referral Process
Contact your local Community Care Access Centre – C.C.A.C. to  determine if you are eligible to receive our services. The C.C.A.C. will help you coordinate a plan that's responsive to you and your caregiver or family support system. The C.C.A.C. are responsible for authorizing admission to Long Term Care Homes.

Long Term Care Facility Rates
The Ontario Ministry of Health and Long Term Care establish Long Term Care Home rates annually. Rates are the same for all long term care facilities in Ontario.
